The draw places Antonio Conte’s Napoli on 26 points, just one ahead of Atalanta, Fiorentina, and Lazio, who sit tied with Inter at 25 points.
Scotland’s McTominay opened the scoring for Napoli midway through the first half, skillfully flicking in Amir Rrahmani’s touch at the near post.
Inter equalized through Calhanoglu, who unleashed a powerful 25-yard shot past Napoli’s keeper, Alex Meret.
As Inter took control, a foul by Andre-Frank Zambo Anguissa on Denzel Dumfries offered Calhanoglu a chance to secure all three points from the penalty spot.
However, the Turkish midfielder, having converted his previous 19 penalties, struck his effort off the left post.
Conte, returning to the San Siro where he led Inter to the 2020-21 Scudetto, will likely be content with a point. Nonetheless, a late attempt from Argentina’s Giovanni Simeone nearly stole the win for Napoli.
